+ it’s necessary to make an appointment for vaccination from 8-00 till 20-00 by phone number 285-07-03 (we will contact with you to coordinate suitable time)
+ come to MBHI city polyclinic № 4

V necessary documents:
+ a passport
+ one of the following documents:
- a patent for labor activity
- a residence permit
- a temporary residence permit
- SNILS (insurance “pension” card if you have)
V documents to fill out and sign before vaccination:
+ a contract for the provision of paid services
+ a voluntary informed agreement for vaccination and processing of personal information
+ a profile

+ the medical service includes: therapist’s examination and single vaccination (the vaccine price is included)
